Peters Accountable Care Organization
Peters ACO’s mission is to provide better care, improve population health, and lower healthcare expenditures in Guilford and surrounding Counties by using the team approach to provide enhanced and coordinated patient-centered medical home-directed care for patients through such things as prevention, standardization of evidence-based best practices, community-based care coordination, investments in health information technology, health information exchange, medication reconciliation, and reimbursement incentives for increasing the quality and efficiency of care for patient populations.
What Is An ACO
An Accountable Care Organization (ACO) is a group of doctors and other healthcare providers who agree to work together with Medicare to give you high quality care. ACOs may take different approaches to giving you coordinated care. Some ACOs may have special nurses that help you set up appointments or make sure your medications are in order when you enter or leave a hospital. Other ACOs may help your doctors get you equipment for monitoring your medical conditions at home, if you need it. Most ACOs use advanced systems that let them more carefully coordinate your care, and make sure your doctor has the most up-to-date information about your health.
The goal of the ACO is to support your doctor in caring for you by making sure they have the most up-to-date information about your health and your care. For you, this means your doctors communicate with each other more effectively, and you avoid having duplicate tests or answering the same questions over and over. Working together, your doctors can do more to follow your health, make sure you get high quality care, and may hire additional staff to help meet your unique care needs, depending on what works for you.
Doctors and other healthcare providers choose to participate in an ACO because they’re committed to providing you with an improved care experience. They may also be rewarded for offering you high quality, more coordinated care. If your doctor chooses to participate in an ACO, you will be notified, either in person or by letter.

I. History
While the problems of fragmented, duplicative, and uncoordinated care affect almost all patients in the United States healthcare system today, these problems are especially acute in central North Carolina. In response to the dire healthcare environment, Peters ACO was formed with the objectives of providing Quality care, improving population health, and lowering the unsustainable growth in healthcare expenditures in a community desperately in need of healthcare delivery reform.
Following Peters ACO’s formation in June of 2019, Peters ACO began recruiting physicians and physician practices in High Point, North Carolina [or Guilford and surrounding counties]. Peters ACO deliberately sought out primary care physicians who would be willing to embrace the cultural transformation required for an effective, collaborative, ACO environment. Peters ACO’s providers have diligently researched accountable care delivery models, have attended ACO training seminars and workshops, and have begun working with health information technology and clinical consultants in order to be as well-positioned as possible to begin participation in the Medicare Shared Savings Program.
II. Mission
Peters ACO’s mission is to provide high quality care, improve population health, and lower healthcare expenditures in Guilford and surrounding Counties by using the Multidisciplinary approach to provide enhanced, collaborative, accessible and coordinated patient-centered medical home-directed care for patients through such things as prevention, standardization of evidence-based best practices, community-based care coordination, investments in health information technology, health information exchange, medication reconciliation, and reimbursement incentives for increasing the quality and efficiency of care for patient populations.
III. Organization
Peters ACO is organized as a limited liability company, intended to be comprised of a network of individual practices of ACO professionals. Peters ACO has an identifiable governing body (known as the “Governing Board”), which consists of [9] ACO participant representatives and one Medicare fee-for-service beneficiary representative. The Governing Board has responsibility for oversight and strategic direction of Peters ACO and for holding Peters ACO management accountable for Peters ACO’s activities as described in 42 C.F.R. Part 425. The day-to-day operations of Peters ACO are managed by Peters ACO’s officers, which include a President, Vice President, Secretary, Treasurer, Medical Director, Compliance Officer, and Privacy Officer. Finally, Peters ACO has established [4] committees, including the Clinical Committee, Information Technology Committee, Finance Committee, Compliance Committee, which are designed to implement the policies and decisions of the Governing Board.
