Form 1095-C Availability Notice

Pursuant to the Employer Paperwork Burden Act, Bethany Medical Department hereby notifies employees Bethany Medical is no longer required to automatically mail a copy of 1095- C to employees.

 

For Current Employees: The Form 1095-C is available in Paycor under Pay Stubs & Tax Docs > Tax Documents.

For Former Employees: The Form 1095-C can be provided upon request at no cost.

Requests may be submitted at any time and will be fulfilled within 30 days of receipt. If you do not request a copy, the form will not be automatically mailed to you but will remain available upon request in accordance with applicable law.
