This is a summary of the annual report of the Bethany Medical Center Employee Benefit Plan, EIN 56-1564485, Plan No. 501, for period 11/01/2024 through 10/31/2025. The annual report has been filed with the Employee Benefits Security Administration, U.S. Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA).
Bethany Medical Center has committed itself to pay certain self-insured Medical, and Dental claims incurred under the terms of the plan.
Insurance Information
The plan has contracts with United of Omaha Life Insurance Company to pay Vision, Life Insurance, Short-term Disability, Long-term Disability, Accidental Death and Dismemberment, Critical Illness and Accident claims incurred under the terms of the plan. The total premiums paid for the plan year ending 10/31/2025 were $321,006.

Statement According to the Paperwork Reduction Act of 1995 (Pub.
